New Influenza Vaccinations

Vaccine inoculation against the New Influenza is available from Jan. 18 (Mon) for anyone including persons who were excluded from the priority order.
Number of Inoculation
(1) Under 13 years old: 2 shots
(2) 13 years and over: 1 shot
* A person whose underlying disease significantly impairs immune reactions may have 2 shots.
Inoculation Fees
1st shot: 3,600 yen
2nd shot: 2,550 yen (separated by at least 1 - 4 weeks)
* 3,600 yen in case 2nd shot administer to another medical institution.
Reservation
Make reservations at medical institutions of your choice after confirming the schedule (for medical facilities, see Nov. 15 issue of City Newsletter [Shi-ho]).
New Influenza Vaccinations (Japanese)
Here is the list of the medical facilities in Nishitokyo City (in Japanese).
At the time of vaccination, bring the following documents to the medical facility
(1) Expectant mothers: Boshi Kenko Techo
(2) Those who have underlying disease: certificate that proves you have priority of being vaccinated (if you visit other than your home doctor, certificate must be obtained from your home doctor)
(3) Infants and students of elementary, junior high and high school: documents that verify your age, such as Boshi Kenko Techo, health insurance card, juminhyo, etc.
(4) Parents or guardians of infants under 1 year old: documents that verify the infants and you are in the same household, such as Boshi Kenko Techo, health insurance card, juminhyo, etc.
(5) Parents and guardians of those given priority but are unable to be vaccinated due to health problem: a certificate that proves you have a priority of being vaccinated (it can be obtained from your home doctor), and other documents such as health insurance card, juminhyo, etc. that proves you are in the same household as those who can not be vaccinated due to health problem.
(6) Others: Health insurance card and the documents that prove your age such as juminhyo, student identity card, driving licence, etc.
Application for Vaccine Inoculation Fee Aid
The City Government provides a full amount aid for vaccine inoculation fee for (1) families receiving the family aid and persons who are receiving the support for the Remained Japanese in China and (2) persons whose any family member on the same resident card is not subject to any city or village tax of 2009.
Application for the aid shall be made before Mar. 31 at the Health and Pension Section of the City checking required documents after inoculation of the vaccine. The persons described in (1) who will have the inoculation within the City shall obtain the Entitlement Certificate from the Social Welfare Section of the City before the inoculation and turn in the certificate to the practicing medical facility. Payment at medical facility or later applications are not needed.
